The states listed below require a state license number to prescribe. If your state is not listed, you can leave this field blank.
Ohio does not require your state prescribing license number to appear on prescriptions. Some health systems, payers or other entities may require this number. If your state prescribing license number is required, enter your license number followed by a space and the letters CTP.
Texas requires your prescriptive authority ID to appear on prescriptions. Enter APRN followed by the ID number. Do not include a space between APRN and the ID number.
Virginia does not require your prescriptive authority ID if the federal DEA number appears on the prescriptions. If you want your state number to be included, enter it in the State License field.
